Everything you need before, during and after your appointment at Advanced Sports & Spine, so you can walk in prepared and leave with a plan.
Four things that make your first appointment quicker and more useful.
Our new patient forms cover your medical history, insurance details, a records release and HIPAA consent. Filling them in at home saves you 15 to 20 minutes in the waiting room.
The more your physician can see, the better the plan. If you have them, bring:
We verify your benefits before your visit and tell you if your plan needs a referral. Bring your insurance card and photo ID. Co-pays are due on the day.
That gives us time to check you in, confirm your insurance and finish any outstanding paperwork. If you're running late or need to reschedule, call us and we'll sort it out.
Plan on about an hour. Here's how it runs.
Our front desk confirms your details, takes your forms and verifies your coverage.
A medical assistant takes your vitals and goes through your history and current symptoms.
Your physiatrist reviews your records and imaging, examines you and pinpoints the source of the pain.
You leave with a non-surgical treatment plan, clear next steps and the same physician for every future visit.
Many procedures can be performed in our own offices, so you won't be sent to a hospital or pay a hospital facility fee.
Anything your physician prescribes is sent electronically straight to your pharmacy. There's no paper script to keep track of.
We bill your insurance directly. Statements and balances are available through our patient portal, and our team can answer billing questions on (704) 542-3988.
You'll see the same physician each time. Follow-ups are usually shorter than your first visit, and we can book them at whichever office suits you.
Three offices, one phone number. Free parking at all of them.
Our FAQ covers insurance, referrals, wait times and what to expect from treatment. If it's not there, call us.
