These terms govern your use of the Advanced Sports & Spine website. Please read them before using the site or submitting a request through it.

This website is operated by Advanced Sports & Spine, an interventional pain management practice with offices in Charlotte and Huntersville, North Carolina, and Fort Mill, South Carolina. In these terms, “we,” “us,” and “our” refer to Advanced Sports & Spine, and “you” refers to anyone who visits the site or submits information through it.
By using this site, you agree to these terms and to our Privacy Policy. If you do not agree, please do not use the site. You must be at least 18 years old to submit a request through this website. A parent or legal guardian may submit a request on behalf of a minor.

The content on this website is provided for general information only. It is not medical advice, it is not a diagnosis, and it is not a recommendation for any particular treatment.
Using this website, submitting a form, or exchanging messages with us does not create a physician-patient relationship. That relationship begins only when you are seen and accepted as a patient of the practice.

Medical emergencies
Do not use this website, our forms, or text messaging to report a medical emergency. These channels are not monitored around the clock and messages may not be seen right away. If you are experiencing an emergency, call 911 or go to the nearest emergency room.
Appointment requests
Submitting a form on this website is a request, not a confirmed appointment. An appointment exists only after a member of our staff has contacted you and confirmed a date and time. We will make reasonable efforts to respond during business hours, Monday through Friday.
We may decline or reschedule a request, and we may ask for additional information before scheduling. Please keep the details you send through a web form or text message general. Do not send medical records, images, or sensitive health history through those channels. If we need that information, we will ask for it through a secure route.
You are responsible for the accuracy of the information you submit, including the phone number and email address you give us.
Communications from us
By submitting a request, you agree that we may contact you by phone, voicemail, or email about that request and about your care.
If you check the text message consent box on one of our forms, you are agreeing to receive text messages from Advanced Sports & Spine relating to your appointment request and your care. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Mobile carriers are not liable for delayed or undelivered messages. Consent to receive text messages is not a condition of treatment, and you may withdraw it at any time.

Coverage and billing questions
Any information on this site about coverage, benefits, or payment is general and does not apply to any individual plan or policy. Coverage depends on your plan, your benefits, and the services your provider determines are appropriate.
